Paulsboro Public Schools has partnered with Inspira Health to host a COVD-19 Vaccine Clinic here at Paulsboro High School. They will be administering the Pfizer Vaccination.

This clinic will be open to all students 12yrs+, staff as well as the general public. 

The date for the first dose is Tuesday, May 25th and the second dose will be Friday, June 11, 2021. Both sessions will be held in the Paulsboro High School Gymnasium.

Those interested in obtaining the vaccine must register in advance and complete either the electronic consent embedded in the registration link below (for those 18 years of age and older) or the attached PDF consent for those under the age of 18 years.  The paper consent must be completed and brought to the vaccination appointment. Additionally you will find the Emergency Use Authorization (EUA) Patient Fact Sheet which must be provided to all parents who are giving consent to vaccinate minors. We will also provide a paper hard copy of the EUA fact sheet on site to all those who are vaccinated.

Those interested in being vaccinated should complete the registration information by clicking the link below.  All information collected in the registration process is captured securely and communicated directly to Inspira Health.  Once registered, representatives from the high school will coordinate the scheduling of students and staff for vaccination on Tuesday, May 25th.  Please register using the link below:

For students that are 18 or older as of Tuesday, May 25th, the consent form is embedded in the registration link above, and no additional paperwork will be required.

For students that are 12 to 17 as of Tuesday, May 25th, a paper copy of the consent form that is attached to this email must be completed and brought to school on Tuesday, May 25th.

All students are asked to bring a copy of their health insurance card, but please note that students will be able to receive the vaccine, regardless of health insurance.

Inspira Health is administering the Pfizer vaccine. Please review the attached Emergency Use Authorization Patient Fact Sheet for information specific to the Pfizer COVID-19 vaccine.  Those electing to be vaccinated will be scheduled for their second dose 3 weeks after their first (June 11th).  Second doses will be scheduled when you arrive for your initial dose and will be conducted on site at the High School.  While the vaccination process itself takes minutes to complete, please expect to wait 15 minutes after your vaccination for your safety to monitor for any side effects.  Those who have a history of allergic reactions for which they carry an EpiPen will be monitored for 30 minutes.
